Indomethacin-responsive headache syndromes

چکیده
Indomethacin-responsive headache syndromes represent a unique group of primary headache disorders characterized by a prompt and often complete response to indomethacin. Indomethacin-responsive headache syndromes are divided into 2 distinct categories in the International Classification of Headache Disorders, 3rd edition (ICHD-3 beta): trigeminal autonomic cephalalgias (paroxysmal hemicrania and hemicrania continua) and other primary headaches, including primary stabbing headache and Valsalva-induced headaches (primary cough headache, primary exercise headache, primary headache associated with sexual activity). In addition, cluster headache and hypnic headache have been described as another 2 primary headache disorders that may respond to indomethacin. The velocity of publications on indomethacin and headache has decreased in recent years. Most papers have been specific to indomethacin and trigeminal autonomic cephalalgias (Bordini et al 2016).

Headache is a common symptom in childhood and adolescence. Effective therapy for this symptom is based on the specific headache syndrome. This article presents examples of the four recognized Indomethacin-responsive headache syndromes encountered in pediatrics including exertional headache, cyclic-cluster migraine, chronic paroxysmal hemicrania, and hemicrania continua. Although uncommon condit...

The case of a man with a 34 year history of episodic cluster headaches is described. At the peak of a cluster the headaches occurred up to twenty times a day. The headaches were unresponsive to conventional therapy but were dramatically abolished by indomethacin. This effect of indomethacin was confirmed in a double-blind placebo-controlled trial.

Atypical features of hemicrania continua (HC), including both visual aura and side shifting, have been reported previously. However, auras and variable unilaterality have never been reported together in HC. We report two patients with side-shifting HC with aura. These patients' symptoms are unilateral headaches, visual aura, autonomic features, throbbing pain, nausea and photo/phonophobia. One ...

Bingel and Weiller (1) report a case of short-lasting, frequent, bilateral, indomethacin-responsive headache without cranial autonomic features in this issue of Cephalalgia. They consider this to be a bilateral form of paroxysmal hemicrania (PH). We not...
